Corrective maintenance is a maintenance task performed by the ICT Services Unit to all computer units in the University to identify, isolate, and fix a fault of a machine or system so that it will be restored to its operational state.

Request for Corrective Maintenance
Client
The client approach the ICT Frontline Service Attendant and ask for a Service Request Form The client fill-up the form and have it signed by his/her immediate supervisor. If the requisitioner is the head of the unit then signature for immediate supervisor is no longer needed.

Information Technology Officer
The IT officer reviews the request and signed for approval.
Scheduling and Issuance of Service Order
Frontline Service Attendant
The Frontline Service Attendant receive the approved Service Request from client and issue a Service Order to concern technician. The Frontline Service Attendant will then log the work order to the information bulletin.
Service Order Form – Corrective Maintenance
Performance of Work
IT Technician
The IT technician receive the Service Order and the equipment to repair. The equipment will be assessed, diagnose/repair and release to client.
End User’s Acceptance
Client
The end-user receives the equipment and attest on the status of the equipment after repair.
